Liability Release and Certificate of Insurance Requirements

Liability Release: We ("Registered Team, Staff and Members") hereby release and discharge The Ohio Cup YOuth Tournament and the US Lacrosse Organization, its officers, coaches, sponsors, and representatives from any and all obligations and/or liability resulting from accidents, injuries, or otherwise occurring as a result of participation in or attendance at the Ohio Cup Youth Tournament.

All teams/players must either be registered with US Lacrosse and it would be the exclusive responsibility of the coach to obtain and provide a certificate of insurance. If team IS NOT registered with US Lacrosse, that program participating must provide a Proof of insurance form from their programs insurer. This too would be the responsibility of the coach to provide to the tournament. If your team is registered with US Lacrosse you can request a certificate of insurance from Bollinger Insurance at http://www.bollingerlax.com.
REFUND POLICY: in the event of a cancellation by a team within 2 weeks prior to date of the Ohio Cup or cancellation due to weather related issues:

1.         No team with receive a full refund. Partial refunds will be issued if the Tournament is
            canceled in whole.
2.         No refunds will be issued if the event is shortened in part for any reason, including
            inclement weather or any other "Act of God".
                a) If teams get only one game, refund would be $200.00
                b) If teams get only 2 games, refund would be $100.00
3.         No refund for any team that is expelled/removed from the event by the Director's for
            using un-sportsmanlike behavior that requires the team removed from the event.

Spectators also need to respect the Officials and the Players. Bad behavior will not be tolerated. Should referees or the Tournament  Director's deem a spectator, player, or coach needs to be removed from the site of the event, it will be the responsibility of the coach(s) from the offenders team to remove the offender from the site. The Director's of the Ohio Cup reserve the right to cancel, shorten, or reschedule games due to inclement weather, poor field conditions, or player and fan safety concerns. Hilliard Youth Lacrosse Association, its officers, employees and staff will not be responsible for any expenses incurred by any team, or organization if the event is canceled in whole or in part. The Tournament Directors along with the hosting facility reserve the right to decide on all matters pertaining to the operation of the event and its judgment is final.

In the event of adverse weather or unplayable field conditions, the Ohio Cup Tournament Board and/or the hosting  facilities reserves the right to:

-   Cancel games and/or remaining schedule in their entirety.
-   Reduce game times in order to catch up with schedule.
-   Finish games before inclement weather arrives, or to preserve field conditions.
-   Reschedule games, if possible.
-   Convert Tournament to Festival format without playoffs or championship
         to provide the majority of the teams the opportunity to play as many
         games as possible.

Every attempt will be made to update cancellations as soon as possible. However, weather conditions can change rapidly and all teams should be prepared to play as soon as weather and fields are playable.

If thunder or lightning is observed, play will be suspended immediately and players and fans will be asked to leave the fields to return to cars. Play will be allowed to resume 20 minutes after the last sound of thunder or sighting of lightning is observed which will be determined by the Tournament staff and the Tournament Officials. Every attempt will be made to begin subsequent games on time.
